Social Service Coordination Director

The role of the Service Coordinator is to coordinate elderly, disabled and low-income individuals to supportive services and community resources. Fosters a housing plus supportive service environment that allows vulnerable persons to live independently and remain in their homes.


  • Assess service needs of residents and coordinate the appropriate community resources or program resources
  • Monitor receipt and follow through of services; including resident satisfaction
  • Supervise, coordinate and deliver vital social services to address the complex needs of residents
  • Responsible for the supervision of any meals programming, including the daily oversight of meal recipients
  • Facilitate internal programs and services such as health and wellness or transportation
  • Develop a resource directory of local social service agencies and providers
  • Assist residents with applications for benefits and entitlement programs
  • Seek to establish and sustain strong coordination that helps residents to identify and effectively manage their preventative health and social service needs


  • 4 years related work with the senior population.
  • Must have knowledge and experience with local, state and county entitlement programs.


  • Bachelor's Degree in the human service field or related area of study.

REQUIREMENTS : Valid New Jersey Driver's License.

Salary: $35,000.00 /year

Eligible for Benefits 90 Days after Employment
