OptumCare Oregon is looking for full time Nurse Practitioner to provide clinical care across a variety of settings and address the gaps in care that are often missed in traditional clinic visits.
The position will involve floating to various clinics and meeting with patients to:
- Document and address chronic conditions (thereby improving documentation)
- Address quality care gaps
- Identify and address factors related to chronic conditions that may not have been recognized and to further work with patients to better achieve optimum health status
- Educate patients about resources available to them within OptumCare Network
- When necessary, link patients to care management or other services
- Primary care clinics (particularly within practices affiliated with OptumCare Network
- Various specialty clinics (exact locations in scope will be determined depending on clinician home base)
- Home visits, long-term care facilities
- Virtual visits
OptumCare Network consists or several employed medical groups, a network of affiliated independent practices, all of which are physician/clinician led communities deeply rooted in their regions. Position Details:
- Monday-Friday, 8am-5pm Full Time Position
- No nights or weekends required
- Variable locations (including home visits)
- See 10-12 patients a day through a range of modalities (office and telemedicine visits) while in the clinic
- See 6-7 patients/day when doing home visits
- Ensure problem lists accurately reflect chronic conditions
- Ensure patients are engaged with their primary care provider
- Educate patients about available resources through OptumCare
Required Qualifications:
- Active, unrestricted Oregon and/or Washington Medical NP license or ability to obtain by start date. Licensure must be unencumbered, free of any open/unresolved disciplinary actions including probation or restrictions against privilege to practice
- Current association certification as a Nurse Practitioner
- Current DEA certificate prior to start date
- Training or experience in gerontology, geriatric medicine and/or chronic care
