+ Position Purpose: The Social Worker / Case Manager plans, organizes and implements social work treatment for clients and families. Assesses the psychological needs of patients and provides intervention to meet those needs utilizing personal, family and community resources. Provides crisis intervention, brief therapy, and case consultation and collaboration services. The Social Worker / Case Manager, as a member of the Population Health Management department, helps fulfill the Memorial Regional Health (MRH) mission of improving the quality of life for the communities we serve, with patient-centered care and service excellence. Essential Functions and Basic Duties: Primary Duties o Assesses patient, client, or family needs and eligibility for services. o Works with clients, families and appropriate community agencies to coordinate and facilitate services for clients. o Participates in planning treatment programs and in dispositional and discharge plans. o Provides individual and group counseling to clients and possibly families. o Maintains family contact regarding client adjustments. o Actively participates in team review of records and cases. o Participates in agency and community planning and education. o Provides selected agency services to identified caseload; refers clients and families to other services as appropriate. o Develops and maintains client case records, plans and reports. o Knowledge of crisis intervention and brief and long term therapy processes. o Knowledge of patient populations' cultural, religious, ethnic, and social systems in interactions, care planning and education. o Knowledge of medical terms, abbreviations, diagnosis, treatment and prognosis and demonstrates this knowledge verbally and in writing. o Skill in providing care appropriate to the age of the patients served in both inpatient and outpatient settings. o Skill in establishing a treatment alliance and engaging the client system in goal setting and prioritizing. o Ability to manage the professional relationship including termination. o Ability to respond to trauma or psychiatric emergency. o Ability to advocate for patients, and negotiate with physician and ancillary staff for patients. o Ability to advocate for patients with insurance companies and CMS for additional services. o Adhere to MRH standardized processes and procedures that create and support efficient and effective continuity of care practices for patients served by the care coordination team. o Participate in "outside the box" thinking and approaches for patients that could benefit from non-traditional treatment plans. o Establish and maintain excellent working relationships with care coordination team members, patients and their families, providers and other medical personnel, community partners, other MRH staff, with an emphasis on building strong teams, improving processes, and providing excellent customer (patient) service. o Attend meetings to stay apprised of organizational communications. Notify the Director of Population Health Management or other senior leadership of staff concerns; participate in resolutions. o Complete reports in a timely manner. o Contribute to and participate in a collaborative, supportive environment among team members. o Maintains prompt and regular attendance. o Attends staff, team and department meetings. o Attends in-services, staffing and other meetings with supervisor's approval. May be appointed to committees. o Performs related work as assigned. Secondary Duties o Demonstrates 100% commitment to performing according to the CHOICE values of MRH and representing the organization in a positive and professional manner. o Establishes and maintains effective communication and good working relationships with all co?workers. o Adheres to MRH and department dress code policies and procedures. o Utilizes initiative; strives to maintain steady level of productivity; self-motivated; manages activity and time. o Completes annual education, training, in-service, and certification requirements; attends departmental staff meetings or reads meeting minutes. o Maintains regulatory requirements. o Maintains patient confidentiality at all times. o Reports to work on time and as scheduled; completes work within designated times. o Actively participates in departmental and facility performance improvement and continuous quality improvement activities. o Strives to ensure continual compliance with departmental, hospital, state and federal regulations and policies. o Follows policies and procedures for infection control, safety, and risk management to ensure a safe environment for patients, public and staff. Work Environment This position operates in an office environment. Physical Demands The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee has the potential to encounter unpredictable and violent clients. The position has mostly an inside work environment and occasional outside work with exposure to weather conditions. Position Type/Expected Hours of Work This is a full-time position. Days and hours of work are Monday through Friday, 8:00 a.m. to 5 p.m. Some nights or weekend hours may be necessary, on an as-needed basis. Travel Travel is primarily local during the business day. No routine overnight travel is expected. Qualifications: Education/Certifications: o MSW, Master's degree in Social Work from a Council on Social Work Education (CSWE) accredited school o Interest in becoming an LCSW desired, but not required. MRH will provide financial assistance for further education, and supervised experience by an LCSW to assist the MRH Social Worker with an MSW achieve LCSW licensure. o Experience in assessing, planning and managing patient care with a minimum of three years of current experience in an acute care hospital or outpatient based experience and a minimum of six months case management, discharge planning, on-site concurrent review and/or home health experience are required. Experience Required: o Demonstrated success working in an integrated environment and working within a cross-functional team. o Outstanding written and verbal communication, documentation, and presentation skills. o Able to apply National Patient Safety goals to care and documentation. Skills/Abilities: o Knowledge of medical practices and patient care. o Ability to work with a Healthcare Electronic Medical Record to review patient data and run reports. o Ability to communicate effectively, both in writing and orally. o Ability to establish and maintain effective working relationships with employees at all levels throughout the institution. o Ability to interpret, adapt, and apply guidelines and protocols. o Ability to identify and resolve problems in a proactive, collaborative manner. o Outstanding customer service skills. o Demonstrated commitment and leadership ability to advance diversity and inclusion. o Strong analytical skills, critical thinking, attention to detail and accuracy, and strong customer service skills.
