Molina Care Connections is a team of Nurse Practitioners dedicatedto providing preventive care services to Molina members across 14 states.****As a CorporateMedical Assistant, you will:**+ **Help at risk patients by processing and following-up on casemanagement and primary care provider referrals**+ **Facilitate care coordination and intervention by sendingelectronic messages/faxes to primary care physicians**+ **Close gaps in health care by preparing patient lab resultletters and following-up on**+ **Fulfill nurse practitioner supply orders and maintainadequate nursing supplies inventory**+ **Perform administrative tasks such as telephone and emailinquiries, follow-up documentation and update patient records**+ **Assists with special projects under the direction of ClinicalIntegration leadership team****Knowledge/Skills/Abilities**+ **Perform timely documentation in the electronic health record**+ **Ability to communicate clearly and effectively.
